Complex Medical / Post-Surgical Care
We provide transitional and long-term care to people with complex medical conditions that require medical management. Most frequently, people requiring complex medical care are recovering from a stroke, heart attack, surgery, or they need help managing the effects of degenerative conditions like Parkinson’s disease.
Our services concentrate on physically reconditioning the patient. We work toward increasing the patient’s exercise tolerance by strengthening essential muscle groups, improving the patient’s oxygen use, and enhancing cardiovascular response to physical activity.
We provide services such as:
Insertion and Maintenance of peripheral IV line
Care and Maintenance of central IV lines
Oxygen therapy and management
Gastrostomy tube (“g-tube, “feeding tube”) care and management
Wound Care
Along with these medical services, we also educate patients about how to manage their condition successfully when they return home after a short–term rehabilitative stay.

Hospice-Related Services
Hospice programs offer a combination of home and facility care for terminally ill patients and support for their family members. Hospice services focus on relieving pain and symptoms (“palliative care”) rather than on seeking a cure.
At Stone Rehabilitation & Senior Living we provide hospice-related services through an approved hospice program
The goal is to help a terminally ill person live his or her last days with as much dignity, comfort, and enjoyment as possible, at home or in a homelike setting. At Stone Rehabilitation & Senior Living, we provide these services through an approved hospice program.
Pain Management
Pain management services are needed by people who are recovering from surgery or a fracture, living with chronic pain from a condition such as arthritis, or terminally ill. For patients in the first two categories, we help them manage their pain with medication.
We closely monitor and manage medication dosages and effectiveness. The goal is to find the lowest effective dosage that affords the patient pain relief. This is a process of continual assessment and adjustment.
Radiology & Laboratory Services
Stone Rehabilitation & Senior Living schedules appointments and coordinates arrangements with appropriate outside providers for any x-ray, blood, or other lab tests ordered by a physician for our residents.
Respite Care
Respite is one of the simplest things that you can do to have an excellent outcome from your procedure. Remember to follow instructions, eat right for recovery, and always ask for help!
Respite stays are available for residents whose caregivers need time off for time limited breaks. This could include vacations, business trips, or personal time.
During a respite care stay, residents receive the same high-quality care as our other residents, in a safe and secure environment. Respite care residents may enjoy all the services and programs we offer during their stay. Respite stays can vary in length depending on the needs of the patient’s circumstances.
Restorative Programs
Stone Rehabilitation & Senior Living’s Restorative program relies on rehabilitation techniques to prevent disability and promote ability in our residents. Restorative care is appropriate for residents who no longer need intensive rehabilitation services, but who still need some rehabilitation to reach their optimal level of independence or to maintain the progress they’ve already achieved.
Our Restorative program is a team effort, at the core of which are our nursing and rehab team. Together, they deliver care that maintains or improves a resident’s functional abilities in an environment that is positive, cheerful, and caring. Patience is particularly important because by allowing a resident sufficient time to perform activities, the caregiver is also helping preserve the resident’s independence and sense of self-worth.
Skin Integrity Management
Maintaining healthy skin is a constant challenge with many skilled nursing residents. There are risk factors that may include a patient’s lack of mobility, incontinence, or inadequate nutrition. Once we identify these factors, we address them in the patient’s individualized care plan.
Rehabilitation Services
The Stone Rehabilitation & Senior Living rehabilitation team develops an individualized plan of care designed to meet each resident’s specific goals and expectations.
We are proud to offer rehabilitative services 7 days per week to maximize individual resident’s progress and recovery. Our rehabilitation team has been together for over ten years, which ensures continuity of care. The staff takes pride in the progress they see in each individual resident on a daily basis. We offer physical, occupational, and speech therapy.
Telephone / Cable / Wi-Fi
Telephone and cable services are available. Each room is pre wired with cable and phone lines. This service is provided from an outside vendor and will be billed accordingly. Wi-Fi is available free of charge to all residents and guests.
